Shana Mallete (FNP)
Dr. Svetlana Khandros is a board-certified family practitioner with years of
experience in the field. He received his Doctor of Osteopathic Medicine degree
from the New York College of Osteopathic Medicine and his Medical Doctor
degree from the Bogomolets National Medical University in Kiev, Ukraine. Dr.
[Name] is also a fellow and chief resident of the Osteopathic Family Practice
Program at St. John's Episcopal Hospital in Far Rockaway, New York.
Dr. Svetlana Khandros is a member of various professional organizations,
including the American Osteopathic Board of Family Physicians and the
American College of Osteopathic Family Physicians. He is licensed to practice
medicine in both New York and Pennsylvania.
With over a decade of experience in the medical field, Dr. Svetlana Khandros
has worked in several hospitals, including St. John's Episcopal Hospital, Beth
Israel Medical Center, and New York Hospital Medical Center of Queens. He is
currently practicing at Health and Wellness Medical Care, PC, where he serves
as a family practitioner. He diagnoses and treats diseases, disorders, and
injuries of patients, provides primary contact and continuous care toward the
management of patient's health, performs diagnostic procedures, and consults
with other medical practitioners to evaluate physical and mental health.
Dr. Svetlana Khandros is dedicated to providing quality health care to his
patients. He believes in the importance of health promotion, disease, illness and
accident prevention, and regularly advises patients and their families on health
care.
Meet Phelan Clancy, MS, AGNP-BC, a board-certified Adult
Gerontology Nurse Practitioner specializing in Oncology. Phelan
received her Bachelor of Science in Nursing from Fordham University
in 2004 and her Master's in Adult Gerontology Nurse Practitioner with
an Oncology subspecialty from Columbia University School of Nursing
in 2014. She was a Rudin Scholar for Oncology during her studies at
Columbia University.
Phelan has been practicing as a nurse practitioner since 2014 at
Southern Wellcare Medical, PC in the Bronx and Blue Medical, PC in
New York City. She provides comprehensive primary care to up to 27
patients per day, managing patients with multiple co-morbidities,
ordering and interpreting labs and imaging, coordinating care, and
establishing relationships with various specialists to create a detailed
plan of care with appropriate medication management and follow-up.
Phelan is also a non-salary faculty member at Columbia University
School of Nursing, where she serves as a preceptor to AGNP
students. In addition to her professional work, Phelan has provided
24-hour palliative care for an oncology patient and worked at
Columbia University Medical Center in Neuro-Oncology and Memorial
Sloan Kettering Cancer Center in Gynecology.
She is a valuable member of our team and is committed to providing
the best possible care to her patients.

Dr. Pramod K Sanghi
MD – Cardiology
Dr. Pramod Sanghi is an experienced Cardiologist with extensive
training in interventional and nuclear cardiology. He is currently
affiliated with various medical facilities, including Wellcare of BX
Medical PC, Rani Medical PC, Blue Medical Services PC, KS
MEDICAL PC, and Southern Wellcare Medical. Dr. Sanghi's past
experiences also include working as a Full-time Solo Practice
Cardiologist in Michigan and a Full-time Cardiology Consultant in India
and various locations throughout the United States.
In addition to his clinical work, Dr. Sanghi has also been involved in
teaching, serving as a Part-time Teaching Faculty at the Department
of Education in Dearborn, MI. Dr. Sanghi is board-certified in General
Cardiology, Interventional Cardiology, and Nuclear Cardiology.
Dr. Sanghi received his Doctorate of Medicine in Medicine from
Wayne State Medical School in Detroit, MI, and completed his Internal
Medical Residency at Emory University in Atlanta, GA. He also
completed a Fellowship in Interventional Cardiology at the University
of Texas in San Antonio, TX. Dr. Sanghi also holds a Master of Public
Health in Public Health from the University of Michigan and a Bachelor
of Science from the same university.
Dr. Sanghi's passion for providing quality patient care and his
extensive training in cardiology make him a valuable asset to the
medical community.
Dr. Elizabeth Ann Kulesza is a licensed physician in the state of New York
with over three decades of experience in the field of medicine. Dr. Kulesza
has extensive experience in managing and providing direct patient care in
various settings, including inpatient and outpatient clinics, nursing homes,
hospice services, and acute care facilities. She has expertise in reviewing
medical records and orders, history and physical examination, and
diagnostic tests such as MRI, CT scan, X-Ray, EEG, EKG, EMG, visual
acuity test, and pulmonary function test.
Dr. Kulesza has supervised and provided direct medical guidance to Nurse
Practitioners, Physician Assistants, Registered Nurses, Licensed Practical
Nurses, and Certified Nurse Aides. She has served as a medical director
by delegating, coordinating, and providing supervision and direction to her
team of healthcare providers.
Dr. Kulesza has worked with various healthcare institutions throughout her
career, including Wellcare of BX Medical PC, Medical Impressions PC,
AGHO Clinic, Silver Star Medical, ARC of Rockland, Distinctive Medical
PC, Oneida Correctional Facility, and Staff Care, an AMN Healthcare
Company. In these institutions, Dr. Kulesza has served in various roles,
including Attending Physician, Medical Director, and Locum Tenens
Physician.
Dr. Kulesza obtained her M.D. from The Autonomous University of
Guadalajara, Mexico, and completed her Fifth Pathway Program of
UMDNJ – Rutgers Medical School, her Internship in Internal Medicine from
Muhlenberg Hospital affiliated with UMDNJ – Rutgers Medical School, and
Junior and Senior Residency in Internal Medicine from Harlem Hospital
Center affiliated with The College of Physicians and Surgeons of Columbia
University.
Dr. Kulesza is dedicated to continuing medical education programs and in-
service training. She also contributes to quality assurance and improvement
programs.

Dr. Elaine Williams DPM
Dr. Elaine Williams-Paulin R. is a highly qualified podiatrist with a wealth of
experience in her field. She has been practicing as a podiatrist at Uptown
Healthcare Management, Inc. since December 2004. Her other clinical
experience includes working as an academic podiatric surgical physician
and certified primary hyperbaric medicine physician at Our Lady Mercy
Medical Center in Bronx, New York, and as a podiatric surgical resident
and chief resident at the same hospital.
Dr. Williams-Paulin R. completed her education at Our Lady Mercy Medical
Center, where she received her primary podiatric medicine residency
training and completed her post-graduate year II. She obtained her Doctor
of Podiatric Medicine degree from Barry University School of Graduate
Medical Sciences and Surgery in Miami, Florida, and a Bachelor of
Science degree from Brooklyn College in New York.
Dr. Williams-Paulin R. is a member of several student and professional
societies, including the Student National Podiatric Medical Association, the
American College of Foot & Ankle Surgeons, and the American
Association of Women Podiatrists. She has held positions such as
delegate, vice president, and national finance chairperson for the Student
National Podiatric Medical Association.
In addition to her clinical work, Dr. Williams-Paulin R. has been actively
involved in extracurricular activities such as the Barry University Podiatric
Conference in Miami, the APMA Annual Conference in the United States,
and the Our Lady Mercy Hospital Lecture Series in the Bronx. She has
also participated in research proposals, including the analysis of foot
problems for the cause of lower back pain.
Ebony Edouard is an experienced and dedicated nurse leader with a
strong clinical and management background in mental health and
pediatric nursing. With a proven ability to stay calm and intervene
during crises, facilitate groups and educational seminars, and
collaborate on multidisciplinary teams, she has built positive
relationships with patients, family members, physicians, and other
medical professionals.
Currently serving as the Administrative Director of Nursing at Cardinal
McCloskey Services in the Bronx, Ebony is responsible for the overall
supervision of nursing staff in OPWDD programs and Children's
Division programs. She collaborates with the Senior VP of Children's
Services and COO to create and implement protocols and procedures
for the Health services division. Ebony also prepares monthly written
reports detailing areas of need and growth for the Health services
division, arranges orientation and ongoing in-service training for new
staff in Health Services, and attends supervisory meetings, in-service
training programs, and leadership meetings.
Ebony has also worked as a Licensed Practical Nurse at Gold Crest
Care Center, where she administered oral and topical GT tube
medications while closely monitoring for adverse reactions, monitored
IV sites and provided wound care, collaborated with the pharmacy to
order and verify medications, and provided reports to oncoming
nurses while delegating assignments according to facility protocol.
Ebony holds a Postgraduate Degree in Family Nurse Practitioner from
Fairleigh Dickinson University-Metropolitan Campus, a Master's in
Nursing from Mercy College, an Associate in Science from Helene
Fuld College of Nursing, and an Associate in Arts from Hostos
Community College. She is licensed as an RN in both New Jersey
and New York and has extensive skills in Microsoft Office, data entry,
payroll, scheduling, training, organizational skills, human resources,
marketing, nursing, medication administration, employee orientation,
triage, and vital signs.
